What You Need To Do
1) Complete a Travel Health questionnaire from reception or alternatively complete our online application form from the drop-down below.
Online Travel Health Questionnaire
2) Review the travel health requirements for each country to be visited from the following dedicated website:
3) Review the travel recommendations from the above website and hand in your completed questionnaire form to reception and make an appointment with the practice nurse as soon as possible and they will assess your travel needs and the surgery will contact you to advise you.
It is important to obtain your travel advice pack and make an initial appointment as early as possible – at least 6 weeks before you travel. Some vaccines may have to be ordered and therefore you may need a second appointment which needs to be at least 2 weeks before you travel to allow the vaccines to work. If your travel is booked at short notice or is unexpected and you are unable to give us a minimum of 6 weeks notice, we may be unable to offer you travel health advice and you will need to go to a private travel clinic. Any non NHS vaccinations will need to be sourced at a private travel clinic including any anti-malarial medication. It is recommended as part of the UK Vaccination Schedule that everyone has an MMR vaccine.
