How to Register

You can register with our practice in two ways: In-person or Online. Please review the practice boundary below before submitting a registration.

Catchment Area

To register in-person

Collect a registration pack from reception.

  • This will include the necessary forms to fill in and return to the surgery. There will also be a short questionnaire to fill in to provide the practice with other important information, and you will be asked to attend the practice for a New Patient Check while we await your notes arriving from your previous doctor. You will be allocated to a named accountable GP who will be one of the partners and who will have responsibility for your overall care, but you may ask to see any doctor of your choice when making an appointment. It can be very useful to us to know your mobile telephone number as well as land-lines. If you would like us to contact you by text or e-mail, please give us your mobile number or e-mail address.
